 ELIGIBILITY: The Ultraslim Big Day Sweepstakes (the “Sweepstakes”) is open to legal residents of the 50 United States, including the District of Columbia, and are 18 years of age or older and the age of majority in their state of residence (19 and older in Alabama and Nebraska; 21 and older in Mississippi) at the time of entry. Void in Puerto Rico, Guam, the U.S. Virgin Islands, outside the state of ohio, and wherever else prohibited by law.  Licensed Healthcare Providers, Customers, Employees, and Contractors of our company (the "Sponsor"), and their respective parent companies, subsidiaries, affiliates, advertising and promotion agencies, and the immediate family members (spouses, parents, children, and siblings and their spouses) of, and/or those living in the same household of each, are not eligible to enter. People who have been a prize winner in any sweepstakes sponsored by the Sponsor during the 2023 calendar year are not eligible. Entrants are eligible to win one (1) maximum sweepstakes per calendar year. Sweepstakes is subject to all applicable federal, state, and local laws and regulations.

Treatment period: The treatment period will expire after the date of the wedding. Treatment period must be completed by dec 21 2025. Treatment is forfeit if Ultraslim Equipment is not available.

 RANDOM DRAWING: The potential Winners will be selected in a random drawing on or about Monday February 19, 2024 from among all eligible Entries received during the Sweepstakes Period by the Sponsor. WINNER NOTIFICATION & VERIFICATION: The potential Winners will be notified via email and/or phone, and will be required to sign and return, within seven (7) days of notification, an Affidavit of Eligibility, a Liability Waiver, and consent to before and after photos to be used for marketing purposes. Winner's name or likeness will not be announced in order to protect their healthcare privacy rights. Noncompliance will result in disqualification and an alternate winner may be selected.  If a prize notification is returned as undeliverable, or if a Winner is found to be ineligible or not in compliance with these Official Rules, the Winner will be disqualified, and the Grand Prize may be awarded to an alternate winner in a separate random drawing. If Sponsor is unable to determine and verify a potential winner after repeated alternate drawings or if Sponsor fails to receive a sufficient number of Entries to correspond to the number of Prizes available to be awarded, Sponsor reserves the right to not award such Prize(s).  PRIZE CONDITIONS: Grand Prizes are not assignable, transferable and may not be substituted except at Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize of equal or greater value at its discretion. Any other incidental expenses on Grand Prize not specified herein are the winner’s sole responsibility. Except where prohibited by law, Entry and acceptance of Grand Prize constitutes permission to use anonymous photographs, and statements for purposes of advertising, promotion, and publicity (including online posting) in any and all media now or hereafter known throughout the world in perpetuity, without additional compensation, notification or permission. Winner must receive treatment at least once each week between 12pm on monday to 4pm on thursday. Winner must schedule all appointments ahead of time and winner must attend all schedueld appointments. Winner is required to cancel appointment in advance with at least 48 hour business notice. If the winner is not compliant in maintaining weekly appointemtns or cancellation policy, or is a no call no show, the sponsor has the right to cancel all appointments, rescind the award and select another winner without warning. If Winner has a medical condition, mediciation or for any other reason is not willing or able to recieve the prize treatments, the prize will be rescinded and given to another. The prior winner will not be awarded any type of compensation.
